If GetOption("pixel_offset") returns, for instance, 20, then that appears to shift everything up and to the right, rather than actually being an inner boundary. Is that intended?
output text offset only pushes away from left and bottom?

Yes, I think the intention was to push text away from the window boundary a bit, for readability. Not to introduce some sort of border.
